For the first time, semaglutide is available as both a daily pill and a weekly injection for weight loss. Brand-name oral Wegovy tablets launched in late 2025, and compounded oral and sublingual formats have been available through telehealth providers. Here's how the formats compare on efficacy, convenience, side effects, and cost.
What's Available in Each Format
| Format | Brand-Name | Compounded |
|---|---|---|
| Injectable (weekly) | Wegovy pen, Ozempic pen, Zepbound pen, Mounjaro pen | Semaglutide vials, tirzepatide vials |
| Oral tablet (daily) | Wegovy pill (1.5mg, 4mg, 9mg, 25mg), Rybelsus | Compounded semaglutide tablets |
| Sublingual (daily) | None | Compounded semaglutide sublingual drops/troches |
Tirzepatide is currently injectable-only in both brand-name and compounded forms. An oral tirzepatide is in clinical development but not available yet.
Efficacy: Does the Format Affect Results?
The pivotal OASIS 1 trial tested oral semaglutide 50mg daily for weight loss (the clinical development dose, higher than the currently approved doses). Results showed approximately 15.1% body weight reduction at 68 weeks — comparable to injectable semaglutide 2.4mg weekly from the STEP-1 trial (14.9%).
At the currently available oral doses (up to 25mg daily for Wegovy tablets), efficacy data is still being established in real-world use. The approved dosing follows a titration schedule: 1.5mg daily for the first month, escalating through 4mg, 9mg, and up to 25mg.
Convenience Comparison
| Factor | Injectable (Weekly) | Oral Tablet (Daily) |
|---|---|---|
| Frequency | Once weekly | Once daily |
| Needle required | Yes | No |
| Storage | Refrigerated (before first use) | Room temperature |
| Timing requirements | Any time of day, same day each week | 30 minutes before first food/drink/meds with ≤4oz water |
| Travel-friendly | Requires cold storage planning | Very easy — pill bottle |
| Dosing precision | Pre-filled pen: exact. Vial: patient-measured | Exact (single tablet) |
The oral tablet eliminates needles entirely — a significant advantage for patients with injection anxiety or active lifestyles that make weekly injections inconvenient. The tradeoff: you take it daily (not weekly), and there are strict timing requirements around food and other medications.
Side Effects
Both formats produce the same primary side effects — nausea, diarrhea, vomiting, and constipation — because the active ingredient is identical. However, oral semaglutide may produce slightly different GI timing since the medication is absorbed through the gut rather than injected subcutaneously.
Some patients who don't tolerate one format well may do better with the other. This is anecdotal rather than trial-proven, but it's a reasonable consideration if GI side effects are a barrier to treatment adherence.
Cost Comparison
| Option | Monthly Cost | Notes |
|---|---|---|
| Oral Wegovy (NovoCare self-pay) | $149/mo (1.5 & 4mg) | Brand-name. Promo through Aug 2026. 4mg offer only until Aug 31. |
