Tirzepatide Access Compared: Who Offers It, at What Dose Ceiling

Tirzepatide — the dual GIP/GLP-1 agonist behind Mounjaro and Zepbound — has become the most requested GLP-1 medication among telehealth patients. Clinical data shows it outperforms semaglutide on average weight loss, and the arrival of compounded and oral formats in 2025–2026 made it accessible outside the brand-name price ceiling that kept many patients on semaglutide.

But not every provider offers tirzepatide, and those that do vary dramatically on format (injectable vs. oral), dose ceiling (some cap at low maintenance doses), and pricing. We audited tirzepatide access across 12 platforms to answer the three questions patients actually ask: who has it, in what form, and up to what dose.

Injectable Tirzepatide: The Widest Access

Eight of twelve platforms in our comparison offer injectable compounded tirzepatide. The pricing spread is substantial — from GobyMeds at $133/month (12-week bundle) to SHED at $299–399/month depending on dose. The biggest differentiators are flat-vs-tiered pricing and dose ceiling.

Flat pricing: Gala

Gala is the only platform where tirzepatide pricing doesn't change as your dose increases. Whether you're on a starting dose or the highest maintenance dose, it's $179/month on the yearly plan ($199/month effective on a 3-month plan). For patients who expect to titrate to higher doses, this is the most predictable cost structure in the market. The $149/month microdosing track is for patients staying at low, sub-therapeutic doses for metabolic maintenance.

Bundle pricing: GobyMeds

GobyMeds' tirzepatide 12-week starter bundle costs $399 total ($133/month). Single-month purchase: $299. No subscription, no auto-renewal — you buy what you need. Brand Zepbound is also available at $1,399. The no-commitment model is unique: every other provider in this comparison requires either a subscription, a plan commitment, or both.

Oral Tirzepatide: The Emerging Format

Oral tirzepatide is new territory. Unlike oral semaglutide — where both brand-name (Rybelsus, oral Wegovy pill) and compounded options exist — oral tirzepatide is currently available only in compounded form from a smaller set of providers. No brand-name oral tirzepatide is FDA-approved yet.

Three providers currently offer oral compounded tirzepatide:

• MadeMed — Sublingual format at $229/month (3mg dose). MadeMed Club membership ($149/year) unlocks $10–20/month discounts. Also offers oral tirzepatide alongside injectable in their lineup.
• Wellorithm — Compounded tablets at $249/month. This is the same provider with the lowest oral semaglutide pricing ($147/month). Tablet format, not sublingual.
• SkinnyRx ⚠️ — Tablets at approximately $299/month. Warning banner required: SkinnyRx has an FDA Warning Letter on file. Also offers sublingual/buccal semaglutide and injectable options.

Sprout (⚠️ also has an FDA Warning Letter) offers dissolvable tirzepatide tablets at $249–299/month, but only in tirzepatide — they don't carry injectable tirzepatide.

Brand-Name Zepbound: The FDA-Approved Path

For patients who want FDA-approved tirzepatide, two pathways exist in our comparison:

• Sesame Care — Prescribes brand-name Zepbound via LillyDirect. Self-pay pricing starts at $299/month for the 2.5mg dose; higher doses are $449/month. Sesame prescribes only FDA-approved medications — no compounded products.
• GobyMeds — Offers brand Zepbound alongside compounded options at $1,399 (standard self-pay before LillyDirect pricing).
• SHED — Brand Zepbound available at $349/month plus a $99 membership fee.

LillyDirect's own self-pay pricing ($299 at 2.5mg, $449 at higher doses) means brand-name tirzepatide is now cost-competitive with many compounded providers' ongoing rates — a meaningful shift from even six months ago.

Who Doesn't Offer Tirzepatide at All

Found and Oak — both of which rank well in our semaglutide comparisons — are semaglutide-only platforms. If tirzepatide is specifically what you and your provider have decided on, these two aren't options. Embody offers injectable tirzepatide but not oral, and their pricing ($129+ intro, rising to $199+ ongoing) puts them mid-pack for value.

The Regulatory Context

The FDA proposed removing tirzepatide from the 503B bulks list in April 2026 (docket 2026-08552). The comment period closed June 29, 2026, and a final rule hasn't been issued yet. If finalized, this would end large-scale 503B compounding of tirzepatide — but 503A patient-specific compounding would continue under existing law. This is the single biggest variable affecting compounded tirzepatide access in late 2026 and 2027.

None of the compounded tirzepatide products listed in this article are FDA-approved drugs. The FDA has not verified their safety, efficacy, or quality. Brand-name Zepbound (via Sesame Care, GobyMeds, or SHED) remains the only FDA-approved tirzepatide option.
