Two Oral GLP-1s, Very Different Rules

For the first time, patients choosing an oral GLP-1 have options. Oral Wegovy (semaglutide pill) won FDA approval in December 2025. Foundayo (orforglipron) followed in April 2026. Both are daily pills. That's where the similarities end.

Efficacy: Wegovy Has the Numbers Edge

In clinical trials, oral Wegovy produced roughly 15% body weight loss vs. Foundayo's 12.4%. For a 220-pound person, that's roughly 5.7 extra pounds lost with Wegovy. That gap is clinically meaningful — but it's based on trial conditions where every patient followed the strict dosing protocol perfectly.

In the real world, adherence determines outcomes. A medication you take perfectly 30 days a month will outperform a theoretically better medication you take 20 days a month. Foundayo's zero-restriction protocol removes the most common compliance failures: forgetting to fast, drinking too much water, eating too soon.

Convenience: Foundayo Wins Decisively

Oral Wegovy's restrictions sound manageable on paper. In practice, they're disruptive:

Both medications produce the standard GLP-1 GI side effects: nausea, diarrhea, constipation, vomiting. Foundayo's label also lists hair loss as a common side effect (≥5% of participants). Oral Wegovy's side effects are well-characterized from years of injectable semaglutide data.

One notable difference: Foundayo explicitly warns about reducing oral contraceptive effectiveness. While all GLP-1s that slow gastric emptying could theoretically affect pill absorption, Foundayo is the first to carry a specific label warning.

Cost: Both Are Premium-Priced

Both are brand-name-only products with no generic or compounded equivalent. Both are covered under the Medicare GLP-1 Bridge at $50/month. Cash-pay pricing varies by pharmacy and program, but expect brand-name pricing in the $800–$1,300/month range without insurance.

For patients who can't access brand-name pricing, compounded injectable semaglutide remains the most affordable GLP-1 option at $99–$299/month.

The scores are close because these are genuinely different trade-offs. Wegovy's extra weight loss and deeper safety data give it a slight numerical edge.
