Generic Liraglutide vs. Semaglutide: Is the Cheap Daily Shot Worth It?
For weight loss, generic liraglutide is usually not the bargain it looks like in October 2026. In the only head-to-head weight trial, weekly semaglutide produced about 2.5 times the weight loss of daily liraglutide, and Novo Nordisk now sells Wegovy for $349 a month cash, which is in the same range as many pharmacy coupon prices for a month of generic liraglutide 3 mg. The cheaper daily shot still makes sense in narrower cases, mainly type 2 diabetes on a tight budget.
Key takeaways
- Generic liraglutide comes in two versions: the Victoza-equivalent for type 2 diabetes (up to 1.8 mg daily) and the Saxenda-equivalent for weight management (3 mg daily).
- In the STEP 8 trial, weekly semaglutide 2.4 mg led to 15.8% average weight loss at 68 weeks versus 6.4% for daily liraglutide 3 mg.
- As of October 2026, Wegovy injections cost $349 a month cash ($199 for the first two months for new patients) and the Wegovy pill starts at $149, while coupon prices for a month of generic liraglutide 3 mg range from about $372 to over $1,200 depending on the pharmacy.
- The diabetes-dose generic is where liraglutide is clearly cheaper: SingleCare quoted $221 for a three-pen pack in January 2026, and GoodRx coupons started around $144.
- Medicare's temporary $50 GLP-1 Bridge covers Wegovy and Zepbound for weight loss, not liraglutide.
What "generic liraglutide" means now
Liraglutide is the older, once-daily GLP-1 drug sold by Novo Nordisk as Victoza (diabetes) and Saxenda (weight). Teva launched the first generic Victoza in June 2024, and Hikma received FDA approval for another generic for type 2 diabetes in December 2024 (Healio).
In August 2025, Teva launched generic Saxenda, the first generic GLP-1 approved for weight loss, at a wholesale price of $1,165 a month versus $1,350 for brand Saxenda (Fierce Pharma). Cipla followed with its own generic Saxenda in February 2026 (Cipla press release).
The two versions are not interchangeable in practice. Victoza's label tops out at 1.8 mg a day for diabetes (Victoza prescribing information). Saxenda's label climbs weekly from 0.6 mg to a 3 mg daily maintenance dose for weight management (Saxenda prescribing information). Semaglutide, by contrast, is a once-weekly injection (Wegovy, Ozempic) or a daily pill.
Efficacy: what the trials show
The cleanest comparison for weight loss is STEP 8, published in JAMA in 2022. It randomized 338 adults with overweight or obesity and no diabetes to weekly semaglutide 2.4 mg, daily liraglutide 3 mg, or placebo for 68 weeks (Rubino et al., JAMA).
For diabetes, the head-to-head is SUSTAIN 10: 577 adults with type 2 diabetes on semaglutide 1 mg weekly or liraglutide 1.2 mg daily for 30 weeks (Capehorn et al., EMJ Diabetes).
| Outcome | Liraglutide (daily) | Semaglutide (weekly) |
|---|---|---|
| STEP 8: average weight change, 68 weeks | −6.4% (3 mg) | −15.8% (2.4 mg) |
| STEP 8: lost 10% or more | 25.6% | 70.9% |
| STEP 8: lost 15% or more | 12.0% | 55.6% |
| STEP 8: stopped for any reason | 27.6% | 13.5% |
| SUSTAIN 10: A1C change, 30 weeks | −1.0 point (1.2 mg) | −1.7 points (1 mg) |
| SUSTAIN 10: weight change | −1.9 kg | −5.8 kg |
Sources: STEP 8, JAMA 2022; SUSTAIN 10, EMJ Diabetes. Placebo in STEP 8 averaged −1.9%.
The gap is large and consistent. The difference in STEP 8 was 9.4 percentage points in semaglutide's favor (95% CI 6.8 to 12.0). One caveat: the comparison between the two active drugs was open-label, meaning participants knew which one they were taking.
Liraglutide's own label data put its weight effect in context. In Saxenda's main 56-week adult trial, average weight change was −7.4% versus −3.0% on placebo (Saxenda prescribing information).

Price: the generic edge has shrunk
When Wegovy cost well over $1,000 a month, a generic daily shot looked like the obvious budget route. That changed in late 2025, when Novo Nordisk cut cash prices for Wegovy and Ozempic to $349 for most doses (Fierce Pharma). Here is how cash prices compare as of October 2026.
| Option | How it's taken | Approx. cash price per month |
|---|---|---|
| Liraglutide 3 mg (Saxenda or its generic, 5 pens) | Daily injection | About $372 to $1,241 on GoodRx's liraglutide (Saxenda) listing; SingleCare quoted $702 for the generic in January 2026 |
| Generic liraglutide up to 1.8 mg (Victoza-equivalent, 3 pens) | Daily injection | SingleCare quoted $221 (January 2026); GoodRx coupons from about $144, quantity not stated |
| Wegovy injection (semaglutide) | Weekly injection | $199 for first two months (new patients, lowest doses), then $349; 7.2 mg dose $399 |
| Wegovy pill (oral semaglutide) | Daily tablet | $149 (1.5 mg), $199 (4 mg), $299 (9 mg and 25 mg) |
| Ozempic injection (diabetes) | Weekly injection | $349 for 0.25 to 1 mg; $499 for 2 mg |

A five-pen box of 3 mg liraglutide is roughly a 30-day supply at the full maintenance dose. A three-pen box of the diabetes version lasts about 30 days at 1.8 mg and longer at 1.2 mg. Wegovy's $199 intro price runs through December 31, 2026, and Novo says it can change its programs at any time (NovoCare).
Put the two weight-loss options side by side. At the low end, a month of generic liraglutide 3 mg costs about what a month of Wegovy costs. The trial evidence says Wegovy produced far more weight loss for that money, and the Wegovy pill undercuts both at its starting doses.
The daily pill changes the comparison too. Someone who wanted liraglutide because they preferred a daily routine, or wanted to start cheap, can now start oral semaglutide at $149 a month. The trade-off is that the pill has its own titration, climbing every 30 days from 1.5 mg to a 25 mg maintenance dose (Wegovy prescribing information).
Daily vs. weekly: more than convenience
Daily dosing means about 365 injections a year instead of 52. That matters for adherence. In STEP 8, 27.6% of people on liraglutide stopped treatment for any reason, compared with 13.5% on semaglutide, and 12.6% versus 3.2% stopped because of side effects (STEP 8, JAMA).
Stomach side effects were common with both: 82.7% on liraglutide and 84.1% on semaglutide reported a gastrointestinal event. The difference was in how many people could stay on the drug, not in whether nausea happened.
Saxenda's label also sets a checkpoint: adults who have not lost at least 4% of body weight by week 16 should stop, since they are unlikely to see meaningful benefit (Saxenda prescribing information). Generic versions generally carry the same dosing instructions as the brand they copy.
When generic liraglutide can still be the right call
- Type 2 diabetes on a strict budget. Generic Victoza-equivalent liraglutide can cost well under Ozempic's $349 cash price (SingleCare quoted $221 for three pens in January 2026), and Victoza's label includes a cardiovascular risk-reduction indication for adults with type 2 diabetes and established heart disease, based on the LEADER trial (Victoza prescribing information).
- Your plan covers the generic but not semaglutide. Formularies differ, and a covered drug at a low copay can beat any cash price. Ask your plan for its tier and prior authorization rules.
- You tolerated liraglutide well before. Some people already stable on liraglutide may prefer not to switch. That is a conversation for your prescriber.
Two situations point the other way. If you are on Medicare and want treatment for weight, the Medicare GLP-1 Bridge (July 2026 through December 2027) covers Wegovy and the Zepbound KwikPen at $50 a month for eligible enrollees; liraglutide is not on that list (NPR via WLRN). And taking the diabetes-strength generic for weight loss is off-label and stops at 1.8 mg, below the 3 mg dose studied for weight.
The verdict
For weight loss, generic liraglutide is rarely worth it in October 2026. It produced less than half the weight loss of semaglutide in STEP 8, more people quit it, and its coupon price often lands at or above Wegovy's $349 cash price. For type 2 diabetes, the math flips: the Victoza-equivalent generic can be the cheapest GLP-1 available, with a cardiovascular indication on its label, though Ozempic lowered A1C and weight more in SUSTAIN 10. Check your coverage first, then compare real prices with your prescriber.

Frequently asked questions
Is generic liraglutide as effective as semaglutide for weight loss?
No. In the STEP 8 trial, weekly semaglutide 2.4 mg led to 15.8% average weight loss at 68 weeks versus 6.4% for daily liraglutide 3 mg.
Is generic liraglutide cheaper than Wegovy?
Not reliably for weight loss. As of October 2026, GoodRx coupon prices for a month of liraglutide 3 mg ranged from about $372 to $1,241, while Wegovy injections cost $349 a month cash and the Wegovy pill starts at $149.
Is there a generic Saxenda?
Yes. Teva launched the first generic Saxenda in August 2025, and Cipla launched another in February 2026. Both are liraglutide 3 mg daily for weight management.
When does generic liraglutide make sense?
Mainly for type 2 diabetes on a budget, where SingleCare quoted the Victoza-equivalent generic at $221 for three pens in January 2026, or when a plan covers the generic but not semaglutide.
