The Non-Response Spectrum

Level 2: Dose-Appropriate Plateau

You've been at a therapeutic dose (1.0mg+ semaglutide or 5.0mg+ tirzepatide) for 12+ weeks and loss has stalled. Possible causes: caloric compensation, medication interactions, thyroid dysfunction, metabolic adaptation.

Level 3: True Non-Response (Rare)

You've been at maximum dose for 16+ weeks, all treatable causes ruled out, and total weight loss is under 5%. This is where genetic factors (PAM variants, GLP1R variants) become the likely explanation.

The Decision Tree

Step 1: Are you at a therapeutic dose? If not → titrate up before concluding anything.

Step 2: Taking medication correctly? (For oral Wegovy: fasting, water limits, timing. For injectables: proper injection technique, rotation sites.)

Step 3: Check for interfering medications — corticosteroids, some antidepressants, insulin, beta-blockers.

Step 4: Get labs — TSH, A1C, lipase, kidney function, vitamin levels.

Step 5: Consider medication switch — semaglutide → tirzepatide (different mechanism), or vice versa.
